Through a series of visits, we analyse data taken from Practice Focus,
and review practice systems and processes, to produce a report and
recommendations. We put particular emphasis on the importance of data
quality, and highlight areas where the practice prevalence is below
expected. Often coding of patients onto registers could be improved,
and we are able to identify patients who are not coded onto registers
but whose notes contain codes suggesting they should be.

Much of the variation in quality between practices is due to variation
in systems, rather than clinicians, and we emphasise the importance of
a systematic approach to quality improvement. For example attention to
recall and review processes, systems around exception coding and
systematic action planning are all important. With the increasing
demand on clinical and administrative time in the practice, working
smarter not harder is the key to success.
