In-Practice Health Check Discrepancies Investigation Process: NHS Lambeth

Ensure that the following criteria are included in the CLINICAL SYSTEM SEARCH:

-       NHS Health Check Completed - 8BAg

-       QRISK2 or JBS code entered within 2 months after the date of the 8BAg code (38DP, 662k, 662l, 662m, 662n)

-       FAST Alcohol screening code (388u) within 2 months after the date of the 8BAg code 

-       Patients must be 40 to 74 years old AT THE TIME of the health check

 

Following checking the search result, the clinical system search figure may still be higher than the Health Check Focus figure.  A closer review of the outstanding patients (in clinical system search and not in Health Check Focus search) will be required to identify other reasons for the differences:

 

Further Health Check Focus EXCLUSIONS:

-          Coronary Heart Disease diagnosis prior to the health check (QOF IHD codes)

-          Chronic Kidney Disease (CKD 3,4,5) diagnosis prior to the health check (QOF CKD codes)

-          Diabetes diagnosis prior to the health check (QOF DM codes)

-          Hypertension diagnosis prior to date of health check (QOF HYP codes)

-          Atrial Fibrillation diagnosis prior to the health check (QOF AFIB codes)

-          Stroke/ Transient Ischaemic Attack diagnosis prior to the health check (QOF STRTIA codes) 

-          Familial Hypercholesterolaemia identified prior to the health check (QOF FHHC codes)

-          Heart Failure diagnosis prior to the health check (QOF HF codes)

-          Peripheral Arterial Disease diagnosis prior to the health check (QOF PAD codes) 

-          Issue of Statin medication within 6 months prior to date of health check (bxi..%,bxg..%,bxe..%,bxk..%,bxd..%)

-          Latest CVD Risk score prior to health check is greater than 20%   (38DP.,38DF, 662m.,662n.)

-          Patient Unsuitable code entered prior to health check (8CM1.%,8CM4.,9NSH.)
