In-Practice Quarterly Health Check Discrepancies Investigation Process: NHS Lewisham

Ensure that the following criteria are included in the CLINICAL SYSTEM SEARCH:

 

-          38B1 – entered in the quarter (NOT including the child code 38B10)

-          JBS risk score – entered in the quarter (Any 1 or more of the following: 662k, 662l, 662m, 662n)

-          Age 40 to 74 year olds

 

Following checking the search result, the clinical system search figure may still be higher than the Health Check Focus figure.  A closer examination of the outstanding patients (in clinical system search and not in Health Check Focus search) will be required in order to identify other reasons for the differences:

 

Further Health Check Focus EXCLUSIONS:

 

-          Previous 38B1 code, prior to the latest, in the last 5 years

-          Hypertension diagnosis prior to date of health check

-          CHD diagnosis prior to date of health check

-          CKD diagnosis prior to date of health check

-          Diabetes diagnosis prior to date of health check

-          Stroke/TIA diagnosis prior to date of health check

-          Familial Hypercholesterolaemia diagnosis prior to date of health check

-          Atrial Fibrillation prior to date of health check

-          Issue of Statin medication prior to date of health check

 

Other Health Check Focus Criteria:

 

-          JBS Risk score code needs to be within 2 months after the date of the 38B1 code

-          Patients must be 40 to 74 years old AT THE TIME of the health check
