• Added facility to generate an XML file for National Reporting of Health Check data

• South Asian flag now selected automatically if the user selects an ethnicity marked as South Asian origin.
• Patients Ethnicity field is now pre-populated from clinical system data

• Upper limit for Total Cholesterol decreased to 9.9 to comply with NHS Health Check Data Set
• Ethnicity Categories converted to use 16+1 descriptions to comply with NHS Health Check Data Set.

• The inclusion of a report to allow PCT staff to monitor the programme and particularly the follow up of High and Very High Risk patients.
• Exclude patients with Familial Hypercholesterolaemia from the call/recall programme.
• The amendment of the Practice Report to provide sub-sections for each type of Health Check Provider.
• Four new reports added:
  • Age Breakdown report
  • Ethnicity Breakdown report
  • Sex Breakdown report
  • Advice/Referrals report

Health Check Provider

• An amendment to the Results page of the Health Check Wizard to provide more comprehensive and systematic method to capture advice, referrals and actions carried out and follow-up actions recommended by health check providers.

Practice

• Read code for No Family history amended from 1269 to 1224 on the Completed Health Check Report.

• Amendments to the CVD risk score algorithm to remove the weighting for obese patients (Lambeth only)
• New option to provide facility to view previous completed health checks
• Add a limit of 250 characters to the Notes field of a completed health check
• Increase the system inactivity timeout from 20 to 35 minutes (Lewisham only)

Practice

• Amend the Completed Health Check report sent to practices to add the additional READ code 38B10 when a health check has been carried out by a third party

• Warning message now displayed on PNL screen if a practice tries to navigate onto a new page without saving.

• Additional information added to patient search screen to explain why a patient is ineligible for a health check – Description now includes; Age restriction, Pre-existing condition, Recent health check.
• New GP referral now generated if Total Cholesterol >=7.5.
• New patient consent description added for '5. I consent to anonymised data collected during this Health Check being shared with other health professionals for research purposes '.
• Premature Family history of CVD revised to only two options; CVD in 1st degree male relative <55yrs, CVD in 1st degree female relative < 65yrs.
• Postcode field now mandatory when adding a new patient.

PCT/PCSS

• New back button included on the patient display screen.
• New automated High risk follow-up letter generated for all un-registered patients with CVD risk >20%.
• New comments box added to Health Check Actioned process to record the action taken.
• New column displayed on Patient List of who postponed the patient.
• 'View Management Reports' feature now added to the Role Type 'Other'.
• Corrected error in email expiry notification that displayed a negative number of days.
